2024 – 01: Transitional Age Youth Housing Program

What is the current availability of housing that is qualified for HUD/Section 8? Is housing already identified for the clients to occupy

This program is intended to be a scattered site. Transition Coordinators will be responsible for securing units and develop relationships with landlords to increase capacity.

We are open to the possibility of a securing ongoing housing for multiple individuals through partnerships with developers and/or ongoing relationships with landlords.

What is expected from the provider as the “liaison” with the landlord

Coaching TAY in asking questions and advocating for needs, and be point of contact for any issues that can’t be resolved directly with the TAY. In addition, they will utilize their relationship with landlords to continue to build availability of rental units.

In addition, working with both landlord and participant to complete all necessary requirements to meet section 8 specifications for voucher status.

What activities are included in the 10 hours/week of support for each client?

We are open to the approved providers ideas and activities. However, examples of the activities could include (dependent on the person’s needs): grocery shopping excursions, budgeting, benefit applications, social and relationship skills to include neighbors and local supports, computer and electronic skills, employment skills, etc.

Please clarify if this is a 2 year, 4 year, or 5 year program.

Individuals may participate in this program for up to 2 years.

The Transition Age Youth Program is funded up to 2028 and may be reviewed for continued funding dependent on success of programming and availability of funding.

Are the services provided considered all-inclusive, or is it supplemental to whatever behavioral health supports are needed (outpatient therapy, D&A treatment, peer support, etc.)?

The services are supplemental to other traditional behavioral health supports.

Can you further differentiate the responsibilities of direct care staff, vs. the coordinator? Can those roles be combined under one person?

No, the roles may not be combined.

Transition Coordinator is intended to be the supervisor of the program, taking a more administrative role including program planning with landlords and housing authorities, fulfillment of section 8 requirements and various voucher paperwork, and being point of contact with TMCA staff for program monitoring and resolution of any issues. The Transition Coordinator will also complete an assessment of the participant and develop a supportive housing plan for the TAY.

Support Worker is intended to work directly with participant on activities of daily living and implement the supportive housing plan objectives. They are then responsible for ongoing refinement of the supportive housing plan. In addition, they will assist with the purchase and set up of the TAY unit upon signing of a lease agreement.

Are the bridge rental assistance funds separate from the funds awarded to the program?

Yes.

Where do the funds come from to establish households for individuals entering the program (furnishing, etc.)?

Contingency funds are allocated to each individual in the program to assist with the cost of the basic household start-up items.

What are the provider responsibilities from the point of intake through discharge?

Does the client enter the provider’s service prior to housing being identified?

Yes, specifically Transition Coordinator services. The TC will assist the client in identifying where they want to reside within our community. This includes preparing client and any identified family supports for the search for housing, assessing any physical needs participants may have, negotiate the reality of their expectations of housing, assessing the long term viability of the housing unit post-program, etc.

How are clients identified/selected for this program?

Clients will be identified through select referral sources due to the small, intensive level of service provided through this program.

Who establishes the exit/discharge criteria from the program?

This will likely be dictated by a combination of the participant and providers residential plan and meeting goals toward full independence. Participants are not intended to participate in the program for longer than 2 years.

If the client violates lease conditions, is evicted, etc., how does that impact their participation in this program?

Should this occur, expectations would be that assistance would be provided in accessing the level of residential support that is required for the TAY.

Is there any expected breakdown between which counties the sites would be located?

Sites are dependent on where the participants wants to live (in Franklin or Fulton County).

When would this program be expected to start?

Upon completion of contract with selected provider and provider having adequate staffing.
