Community Navigators

Reducing Healthcare Disparity

In 2022, TMCA began partnerships with local Community Based Organizations to encourage the use of preventative services, mitigate social determinants of health barriers, and reduce healthcare disparities that improve behavioral health outcomes. Social Determinants of Health are needs related to childcare access and affordability, clothing, employment, financial strain, food insecurity, housing instability, transportation, and utilities.

We currently partner to provide staffing support for South Central Community Action Program, Center for Community Action, Waynesboro Community and Health Services and Fulton County Family Partnership. Staff members are assigned to the respective programs as “Community Navigators” and connect Medicaid eligible individuals to resources to meet their basic living needs. In meeting those needs, Navigators work on a budget and money plan with each of the participants. This builds the foundation for self-determination and stability in their journey moving forward.
